Adult Learner CE Course Selection Form 21-22 (2)

This form is for Adult Learners. If you are attending high school, you will need to book a time with your grade level Counsellor to cross-enrol to CE.

*Thank you for your interest in SD42’s Continuing Education school. Adult Learners please read and complete the form carefully as you are agreeing to several items when you submit. If you are currently in high school, please contact your school counsellor to discuss taking courses at CE.

We offer morning, afternoon and evening classes.

  • Morning classes: 9:00AM to 12:00PM
  • Afternoon classes: 3:30-6:00PM
  • Evening classes: 6:30PM to 9:30PM

    Supporting Documentation

    To complete the registration process, we require the supporting documentation listed here.

    • A copy of your primary Identification (e.g. birth certificate, passport, citizenship card, Canada immigration documents).

    • Proof of address (BC Driver's License, BC Services Card, mortgage document, municipal tax bill, utility bills).

    You can submit these supporting documents by using the following options.

    1. Scan and Upload using the upload option below

    2. Scan and email the required documents to ce@sd42.ca

    3. Submit the required documents directly to the school (by appointment only). Contact us 604-466-6555 Ext. 1 to set up a mutually convenient time to bring in your documents to our location.

    Agreement

    The School Code of Conduct applies to all courses. Academic Honesty is integral to learning. Teachers and Administrators will investigate all concerns regarding plagiarism which may result in a mark of zero or withdrawal from the course. I understand and agree that after becoming active in the course (10 hours of the course attended) I do not qualify for a refund, but can transfer my fee (one time only) to another course.

    Next Steps

    Once you submit this form, the process is:

    1. a confirmation notice is sent to the student email entered on this form indicating that the registration form has been successfully submitted

    2. a digital copy of your form is directed to the CE clerical team

    3. Students will ONLY be contacted if there is a change to the schedule or if there is a problem with the registration. Please make note of your class start date and time.
